A copy of insurance card front and back along with a copy of a photo ID is required to schedule a consultation.

Note: We are in-network with PacificSource, MODA, OHP, Aetna, and BCBS. We provide superbills for cash pay, and also offer a sliding scale for clients who need financial assistance on a limited basis. Please reach out with any questions, we am happy to work with you.
